Patient Healthcare Advocacy

Patient Healthcare Advocacy

How can a Patient Healthcare Advocate work for you?

MN Senior Care Partners provides Patient Helthcare Advocacy & Elder Services. We do not take over your health care.  As patient advocates we provide a knowledgeable set of eyes and ears to help you understand the care you are receiving, support your wishes and decisions and help prevent mistakes in your health care.  We work for you, not a hospital or an insurance company.

holding handsWe offer Patient Helthcare Advocacy to you by:

  • Keeping communications clear between you, your physicians and other health providers.
  • Coming with you to medical appointments to ensure understanding and prevent misinformation.
  • Organizing your medical records to keep track of test results, medication use, treatments and procedures to prevent duplication of services.
  • Being the primary point of contact to keep all family members informed and updated about your care.

Having a Patient Healthcare Advocate can save you money on your healthcare costs, improves the quality of your elder care and can help you utilize the healthcare system more effectively and efficiently.

We advocate for YOUR healthcare needs. Whether your loved one is looking for home care or alternative housing services, we can provide trusted, professional guidance throughout the process.

Elder care is a challenging area to navigate on your own. Whether your loved one has Alzheimer’s disease or another form of dementia, or perhaps more physical needs that they are battling, reach out to MN Senior Care Partners and we can be your personal Patient Advocate for any healthcare needs.

Advocacy For Patients & Families
Our Geriatric Care Managers ask doctors the tough questions and fight for your loved one. Our foremost responsibility is to advocate in the best interest of your family. Advocacy includes regular communication with physicians and primary medical providers who have great influence over your loved one’s quality of life.

Ongoing Management & Coordination of Care
Some patients may only require an assessment, which provides a roadmap for future care. Others may need ongoing care management. Some may choose to use a care manager a few hours a month while others will do so for specific services or more complex issues only. Many caregivers request that care managers coordinate all of the patient’s medical and social needs to alleviate stress.

“Age In Place” Solutions
“Age in place” has become a prominent phrase as baby boomers age and long-term assisted living housing is at a premium. Age in place simply means finding a way for older adults to stay at home rather than moving to a long-term care facility. However, our care managers are well versed in technology, home modification, home care, and durable medical equipment solutions that often offer a viable option to seniors who prefer to age in place.

Schedule & Accompany Patients To Appointments; Accurately Relay Information
Though families living at a distance from their aging loved ones often benefit from a local Care Manager, local caregivers also see the same benefit from the presence of a healthcare advocate at patients’ medical appointments. Our care managers are then able to succinctly convey that information to concerned family members.

Liaison Service For Families At A Distance Or Otherwise Unavailable
Families living at a distance or otherwise unavailable are even more susceptible to communication problems with medical providers and often feel “out of the loop.” We help coordinate local elder care in the greater Minneapolis area by bridging that gap by communicating with providers, scheduling and accompanying patients to appointments, and relaying information to family members.

Mediate Family Disagreements Regarding Care Options
Caring for aging loved ones creates inordinate stress on family members. Our senior health care advocates can provide objective, expert opinions regarding care options, which often can mediate disagreements among family caregivers.
